Chronic Disease Management
Patients with chronic disease conditions like Diabetes, Arthritis, COPD, or Renal diseases often experience a dip in quality of life due to inability to manage their condition. Skilled nursing services can support them with this and also with reducing medical complications and hospital re-admissions.
KEFI’s Chronic disease management program is patient centric – helping them develop independence, thereby improving their quality of life by a great deal. We leverage the vast knowledge and experience of our medical and nursing staff to design appropriate care plans for patients. Our programs are individually devised using a combination of clinical, physiological and economic factors to achieve the best results. We have developed protocol based care in conjunction with the primary physicians for many including but not limited to the following:
- Balance care
- Wound care
- Dementia care
- Alzheimer’s Care
- Diabetes & Hypertension Care
- COPD at Home
- Heart Disease Care
- Pain Management
- Chronic Kidney and Liver diseases
Normal balance is the hallmark of good health. Conditions like diabetes, high blood pressure, stroke, heart problems, inner ear problems and intake of many commonly prescribed drugs can affect your balance. Loss of balance resulting in fall is an extremely dangerous event with serious ill effects on health. Fractures due to falls can cause loss of independence, long rehabilitative periods and medical complications. The good news is that falls due to loss of balance are largely preventable.
At KEFI, we have a specially trained workforce involving doctors, nurses and physiotherapists to tackle the issues of loss of balance. A full medical assessment, including appropriate laboratory tests and an evaluation by a skilled physiotherapist, forms the cornerstone of our treatment approach. Periodic evaluation by the team over several weeks will help restore balance in patients and help them regain their lost confidence.
Wound healing is a complex process which requires specialized care to prevent any chance of infection. Proper cleaning and dressing of the wound can accelerate the healing process.
At KEFI, we have selected and trained nurses and doctors who are specialized in this area. Wound care, also requires coordination with the other physicians, for instance, the diabetologist, to monitor sugar levels, or other factors that could affect the treatment plan. After taking all of this into account and liaising with the patient’s primary physician, KEFI provides comprehensive care in the comfort of your home.
Our caregivers are trained to assist in the recovery from acute surgical wounds, trauma wounds, burns, chronic wounds, pressure injuries and infected wounds. Our aim is to improve the patient’s comfort, accelerate healing and ensure that no further complications arise.
Diabetes is a widespread condition affecting all age groups. While there is currently no cure for diabetes, the condition can be effectively managed with medication, controlled diet and regular exercise. Complications arising from diabetes range from nerve damage, cardiovascular disease to kidney failure.
KEFI Home Healthcare aims at managing the patient’s condition effectively to prevent further complications and help them improve their quality of life.
Our diabetes care program helps with the following:
- Understand the disease and what causes it to have greater or lesser impact on an individual’s health and well-being
- Know-how to monitor the disease and what to do when levels aren’t in the desired range
- Medication management
- Lifestyle and nutrition education
- Appropriate exercise regimen
- Educate the patient to watch for signs that their diabetes might be having a negative impact on skin or nerve feeling
- Liaise with the primary physician and provide necessary medical care as and when required
COPD (Chronic Obstructive Pulmonary Disease) mainly consists of progressive lung diseases such as chronic bronchitis and emphysema. Due to obstructive nature of the condition, patients may experience difficulty in breathing. Pain alleviation is often neglected in the pursuit of other medical outcomes and the patient is left to battle pain without any recourse.
KEFI Home Healthcare follows a holistic approach actively involving patients in chronic pain, their immediate caregivers, skilled nurses and other disciplines based on the patient’s condition. Pain affects patients due to various conditions including arthritis, cancer, fibromyalgia and multiple sclerosis, among others. We offer the patient a pain management assessment that helps gather information to identify proper methods for treating the discomfort. Our treatment methods depend on the type of pain, its severity, and other external factors that could cause pain to fluctuate.
One of our main priorities is to identify and teach our patients methods and techniques that can control their symptoms in order to help overcome their pain and be independent.
At KEFI, We also provide additional counselling and support, observation and evaluation, and information on medication management, nutrition, and other therapeutic options that may improve their condition.
